CANCELLATION POLICY

Clients who wish to cancel their scheduled appointment are asked to cancel at least 24 hours in advance. 

NO SHOW POLICY

Clients who fail to show up to a scheduled appointment without 24-hour notice, must pay for future scheduled services at the time of booking. If client fails to show up for a second appointment without 24-hour notice, they will not be reimbursed for pre-payment.

Client Intake Forms and Policy Forms can be filled out online through Simple Practice at the time of booking an appointment. It is preferred that forms be filled out online through the Client Portal. If for some reason you are not able to fill out forms through the Client Portal, please download these forms and complete prior to appointment. If bringing printed forms to an appointment, please arrive fifteen minutes early in order for forms to be processed.
